Flight Crews Have Highest Cancer Risk From Radiation of Any Profession

Out of more than 500 occupations studied, two came out on top for radiation-related cancer deaths. Not nuclear technologists. Not X-ray technicians. Flight attendants and pilots.

A major new study published August 17 in JAMA Internal Medicine analyzed nearly 12.7 million deaths across 503 different professions. When researchers looked specifically at deaths from radiation-linked cancers — including melanoma, breast cancer, and other malignancies known to be associated with radiation exposure — flight attendants ranked first and pilots ranked second out of every occupation included.

Flight attendants had roughly 50% higher odds of dying from a radiation-related cancer compared to the general working population. Pilots had about 36% higher odds. In terms of actual numbers, about 6.9% of flight attendant deaths and 6.7% of pilot deaths were from these specific cancers — the highest percentages across all professions studied.

"Based off of prior studies, I expected flight attendants and pilots would have an elevated proportion of cancer deaths, but the rank order — seeing those occupations above all others — was pretty surprising," said lead author Dr. Vishal Patel, a physician at Brigham and Women's Hospital and researcher at Harvard Medical School.

What They're Being Exposed To

The culprit is cosmic radiation — energy that streams constantly from space. At ground level, the Earth's atmosphere absorbs most of it. At 35,000 feet, that protection is dramatically reduced. People who spend their working lives at cruising altitude are accumulating radiation exposure that the rest of us simply aren't.

A regular air traveler who takes 10 cross-country roundtrips a year receives about 0.4 millisieverts of cosmic radiation annually. Pilots and flight attendants, depending on their routes, are exposed to between 3 and 6 millisieverts per year — up to 15 times more than a frequent civilian flyer. The difference is cumulative. Years of working at altitude adds up in ways that occasional flying doesn't.

The study found elevated risk specifically for radiation-linked cancers — not for cancers generally. Neither flight attendants nor pilots showed higher-than-normal death rates from cancers with no connection to radiation. That specificity is what makes the findings so compelling.

"The fact that this was specific to radiation-related cancers was really convincing," Patel said.

What It Means for Passengers

Before anyone cancels their flights — Patel was explicit. "This really is a story about cumulative occupational exposure, not just routine civilian travel. It's not something that the public should worry about."

The risk comes from spending the bulk of your working hours in the sky year after year. A passenger taking even frequent flights accumulates a fraction of the exposure a crew member does. The math only adds up at the occupational level.

A Regulatory Gap

Here's the thing that makes this finding particularly striking. Nuclear technologists — people whose entire job involves working around radioactive materials — have formal occupational radiation protections. Monitoring, exposure limits, and health tracking are standard parts of the job.

Pilots and flight attendants don't have equivalent federal protections for cosmic radiation exposure. The FAA classifies aircrew as radiation workers in its guidance documents, but there are no mandatory exposure limits or routine radiation monitoring for flight crews in the United States. The researchers noted this gap directly, concluding that the findings "support consideration of occupational radiation protections for aircrew members."

Two independent radiation experts from the University of Queensland and the Australian Radiation Protection and Nuclear Safety Agency, writing an accompanying commentary in the same journal, put it plainly: the elevated cancer risk for crew members "supports the hypothesis that occupational radiation exposure, rather than lifestyle or socioeconomic factors, underlies the observed excess."

Industry groups representing pilots and flight attendants have pushed for more formal protections for years. This study gives that push significantly more weight.
